I've been on two road rides with this baby attached and it does exactly what it's supposed to!
Feels very solid on the bike (I have it mounted to the stem, actually, rather than the handle bars. The phone holder part comes off for charging or when you don't want to use the whole rig.
The zip-tie attachment had me a bit worried, but I've not experienced any of the rotation that some of the other commenters have noted. Perhaps that might happen in time.
I'm also thinking that this probably wont fit an iPhone 4. I realize it says that it's for the 3g and 3gs, so that's not a surprise. The phone holder itself is very much custom fit to those phone models.
All in all, a great $15 investment!!
****EDIT****
During a 50 mile ride a week or so ago, I stressed the limits of this mount and came out the loser. While moving around 12mph up a small hill, I rode over a metal cattle guard in the road which caused my old iPhone 3G to rattle loose, fall to the ground, and crack the screen.
Now, while I'm bummed about the circumstance, it's pretty easily avoidable. I still feel like this is an excellent product, but have moved my rating down to 4 stars because it's certainly not perfect. Under normal, smooth, road conditions, this mount will do just fine. I think I'm going to see if I can adapt the mount for a more secure fit. Since I've upgraded to a new iPhone, I'll continue to use "retired" version so maybe a little epoxy can more firmly secure the phone to the mount.

I've been biking with this for abt a month now and it works perfect.
Iphone stays intact on bumpy trails, no issues.
I would recommend this product.

Used it twice so far - mostly on roads, but definitely hit a few decent bumps. Never loosened or seemed like the iPhone would fall out. Rather than mounting in the center, I did it over to the side, near the handle, which seemed to make it easier to use the iPhone cyclocomputer app while riding - less range of motion to hit a button. I did leave some lag on the straps, just in case it loosens up after some settling/usage, but it seems really tight on the handlebar for now. The neoprene adhesive sheet that it comes with allows you to get a very good grip on the handbars.

Easy to install, easy to use. I've been using it for about 3-4 weeks and my iPhone hasn't hit the pavement yet, so I believe this mount is doing its job.

Works great so far. I can tell the top clip may not last too long if you pull it far back alot to take your phone off and on but its great for the price.
